Sick & Accident
How to request a sick & accident claim form:
Please contact Ironworkers Local 377 within 30 days of your injury/illness.
The office will mail you a yellow sick & accident claim form. You will fill in the top section and the licensed physician will need to complete the middle section to certify your injury/illness.
Upon completion of the form you will then need to mail it the San Francisco Hall at: 570 Barneveld Avenue, San Francisco, CA 94124.
The Sick and Accident Committee reviews these forms at member meetings on the 2nd Friday of every month. The Sick and Accident Committee will decide if you are eligible for Sick and Accident benefits. Your dues must be current at the time of the accident and current at the time of the request.
